Service claimed
MBS item 302 - consultant psychiatrist, referred consultation, consulting rooms. (Professional attendance by consultant physician in the practice of his or her specialty of psychiatry where the patient is referred to him or her by a medical practitioner). An attendance of more than 15 minutes duration but not more than 30 minutes duration at consulting rooms, where that attendance and any other attendance to which item 300, 302, 304, 306 and 308 apply have not exceeded the sum of 50 attendances in a calendar year.
When asked about what history was taken, although the practitioner said he could not recall the consultation, he stated:
'would have asked her about why she missed her - been missing her appointments with me, and hence the issue about her moving house'.
He went on to say:
'I wrote down "moved house" and "moved house" would have been [the] answer to a question, so the inference about the kind of question I would have asked would have been "Why have you missed two appointments in a row." [name] and she - and I haven't written that down. I've written down her answer "moved house", says stabilised now.'
